In order to enroll your child at Shining Stars Child Care, Inc. you must complete the registration form online or in person, in one of two ways.

Complete the registration form.

(You will complete and sign the enrollment forms when you visit our center to complete enrollment)

Regardless of how you complete the forms, to finish the enrollment process you must come in person to Shining Stars Child Care, Inc. At that time the Provider will complete the enrollment process and review the policies set forth in the Parent Handbook.
